Care Navigators

Care Navigators are part of the NHS. They are here to help people with more complex health and care needs, such as people who have just been discharged from hospital or who are unable to leave their home. 

What do Care Navigators do?

Care Navigators help people who have more complex health and care needs to access the right services they need to live independently at home, for example Adult Social Care.

Care Navigators can act as a central point of contact for patients and their relatives / carers when they need advice.

They can also help you navigate different healthcare services, follow up on queries for you and help you make appointments.

They can also do home visits if needed.

After your first meeting with your Care Navigator, you will agree on an action plan where you can share what is important for your health and wellbeing.
